Job Requirements

• MGB Badge Photo: Submit a color photo taken in past 6 months with plain background, no selfies or filters.
• License/Certificate Verification: Primary Source Verification of professional license and/or certification, as required for the position, within 30 days of start date and updated upon expiration. MA Conditional Licenses not accepted.
• Fit Test: N95 fit test within 365 days to start date and updated annually. Must be completed on the MGB N95 Respirator Fit Test Record Form.
• MMR: Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity via blood test.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions or waivers accepted.
• TB Test: Documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or negative QuantiFERON TB Test or T-spot within 3 months and updated if >6 month break.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
• MGB Criminal Background Check: Within 45 days prior to start date and updated if >6 month break.
• MGB Massachusetts CORI: Background check within 30 days prior to start date and updated if >6 month break.
• MGB Resume: Include past 7 years of work experience with city and state.
• MGB References: 2 recent references, minimum 1 supervisor or above, in skillset.
• MGB Skills Checklist: Required for submission.
• MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions upon acceptance of assignment.
• MGB Education Verification: Verified through background check company, must verify highest level education achieved.
• MGB SSN Trace: Must be run with criminal check and list addresses/aliases for past 7 years.
• MGB National Sex Offender Check: Ran within 30 days of start and updated annually.
• MGB Exclusion Checks: Ran within 30 days of start and annually, includes OIG, GSA, MassHealth & NH Medicaid exclusion lists.
• MGB Candidate Submission Coversheet: Use current version found in Document Library, required for submission.

Additional Details

• Hepatitis B: Strongly preferred, record of 3 vaccines or proof of immunity or declination.
• Tdap: Vaccine within 10 years and updated upon expiration preferred.
• Positive PPD - Negative Chest X-Ray: If positive TB test, documentation of chest x-ray indicating no active tuberculosis is required.
• MGB TB Symptom Survey (if Positive PPD): TB screening form to determine no symptoms suggestive of TB infection must be completed prior to start and updated annually.
